1994-047 PBA - Lieutenants Contract1 RESOLUTION 47-94 A RESOLUTION OF THE VILLAGE COUNCIL OF THE VILLAGE OF NORTH PALM BEACH, FLORIDA, AUTHORIZING AND DIRECTING THE MAYOR AND VILLAGE CLERK TO ENTER INTO A WRITTEN AMENDMENT ATTACHED AS EXHIBIT "A", TO THE 1992-1993 POLICE BENEVOLENT ASSOCIATION-LIEUTENANTS CONTRACT WITH THE VILLAGE; AND, PROVIDING FOR AN EFFECTIVE DATE. BE IT RESOLVED BY THE VILLAGE COUNCIL OF NORTH PALM BEACH, FLORIDA: Section 1. The Village Council of the Village of North Palm Beach does hereby approve the written amendment to the 1992- 1993 Police Benevolent Association-Lieutenants Contract with the Village. Section 2. The Mayor and Village Clerk are hereby authorized and directed to execute the written amendment set forth in Exhibit "A" to the 1992-1993 Police Benevolent Association- Lieutenants Contract for and on behalf of the Village of North Palm Beach. ' VILLAGE CLERK 1 '- Amendment to the Agreement betaeea The Village of North Palm Seaoh and The Palm Seach County Police Benevolent Association (Lieutenant) This Amendment to the Agreement between the Village of North Palm Beach and the Palm Beach County Police Benevolent Association (Lieutenants), is effective the let day of October, 1993, by the employer, THE VILLAGE OF NORTH PALM BEACH, a municipal organization, hereinafter referred to as "VILLAGE" and the PALM BEACH COUNTY POLICE BENEVOLENT ASSOCIATION, hereinafter referred to as "ASSOCIATION". 1leutenante ARTICLfi 6. HEALTH INSURANCE Effective October 1, 1992, the VILLAGE agrees to pay one hundred percent (100$) of the medical, dental and hospitalization insurance premiums for the employee and seventy-five percent (75~) of the employee's dependents for the duration of this Agreement. For the fiscal year 1992-1993, the VILLAGE agrees to reimburse employees all monies they pay for dependent den'..al and hospitalization insurance premiums. The PBA agrees that employees will pay fifty percent (50g) of the cost of dependent dental and hospitalization insurance premiums, such cost to be implemented upon implementation of the new pay plan for the 1994-1995 fiscal year which is to be negotiated between the parties. Such pay plan will contain a structured basis to provide the opportunity to reach the maximum rates per classification as set forth in the Wage and Pay Plan Study by Cody & Associates, Inc. A $200.00 per, person deductible will apply to medical and hospitalization claims and a $100.00 per person deductible will apply to dental claims, subject to restrictions and qualifications of the self-insurance plan. The VILLAGE shall self-insure for all maternity benefits. it will be the responsibility of the employee to notify the VILLAGE within three (3) days in the event that dependant coverage is no longer required due to a change in marital status. Should the employee not notify the VILLAGE of said change, the employee shall reimburse the VILLAGE for the amount paid for his dependent insurance coverage premium from the date of status change. The VILLAGE retains the right to determine the insurance carrier or it may self-insure if it so desires. 1lautenante ARTICLE 29. WAGES All employees covered by the collective bargaining agreement shall receive for the fiscal year 1993-1994, a four (4~) percent increase in their base pay rate. INCREMENTS - the following increments per year may be received by employees after proper accreditation: PARAMEDIC $2,000.00 SMT $1,000.00 FIRS FIGHTER $1,000.00 IN WITNESS WHEREOF, THE PARTIES HAYS SET THEIR HANDS AND SEALS THIS 28th day of October 1994.
