A RESOLUTION OF THE VILLAGE COUNCIL OF THE VILLAGE OF
NORTH PALM BEACH, FLORIDA, CONFIRMING AND APPROVING A
PROPOSAL FROM KWIK ROOTER INC. D/B/A KWIK PLUMBERS TO
PERFORM EMERGENCY REPAIRS TO THE OSBORNE PARK BUILDING
WASTEWATER LATERAL; AND PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the restrooms in the Osborne Park Building have been clogging up, resulting in the
flooding and closing of the Building; and
WHEREAS, the Public Works Department discovered that the clogs and flooding resulted from
heavy scaling to the wastewater lateral and obtained a proposal from Kwik Rooter Inc. d/b/a
Kwik Plumbers to line the existing cast iron pipe to correct the issue; and
WHEREAS, emergency repairs of more than $2,500 up to $25,000 require Village Manager
approval and confirmation by the Village Council; and
WHEREAS, the Village Council determines that the adoption of this Resolution is in the best
interests of the residents and citizens of the Village of North Palm Beach.
NOW, THEREFORE, BE IT RESOLVED BY THE VILLAGE COUNCIL OF NORTH PALM
BEACH, FLORIDA, as follows:
Section 1. The foregoing "whereas" clauses are hereby ratified and incorporated herein.
Section 2. The Village Council hereby confirms and approves a proposal from Kwik Rooter
Inc. d/b/a Kwik Plumbers to perform emergency repairs to the Osborne Park Building
wastewater lateral at a total cost of $13,767.84, with funds expended from Account No. A5519-
34620 (Facilities — R&M Buildings & Grounds). A copy of the proposal is attached hereto and
incorporated herein.
Section 3. This Resolution shall take effect immediately upon adoption.
PASSED AND ADOPTED THIS 13TH DAY OF FEBRUARY, 2020.

SCOPE
1. SAFETY: We will provide orange safety cones and yeltow caution tape for barriers
where the pipe lining process is taking place.
2. ROBOTIC REINSTATEMENTS: We will use our robotic cutting tools to open
branch lines that were lined over. In the event that our equipment will not navigate a
portion of your plumbing system then we will gap the fittings.
3. MOBILIZATION: Owner will provide adequate parking for fetters, box trucks, etc.
4. WATER SHUT DOWN: During the cleaningldescaling and pipe lining process for
the horizontal main Iines the water will be shut down that will be affecting those units.
Water will be shut down from 8:30 am until job completion for that day. UNDER NO
CIRCUMSTANCES WILL ANY WATER BE ALLOWED TO BE. USED
INCLUDING FLUSHING ANY TOILETS, ANY WATER THAT IS USED AT
CURING TIME MAY RESULT IN A COLLAPSE OF NDN -CURED LINER.
5. Any unforeseen pipe problems that may arise during the course of the job will be
brought to the attention of the owner and addressed accordingly. The estimate is
based on the plumbing being functional and in structural condition with no collapsed
sections of pipe. If any of our equipment is lost or stuck in the existing pipe then we
reserve the right to retrieve our equipment.
8. PERMITTING AND PERMITTING COSTS: Casts are not included. Special
engineering drawings or letters that may be required by the Building Department are
not included.
9. Horizontal building drains, horizontal fixture branches, vents, re -vents, or water
closet 90 bends and risers are not included unless outlined in this estimate.

10. WARRANTY: All pipe Iined has a 10 year labor and materials warranty with a 50
O ° o 75 year life expectancy.
I 11. PAYMENTS: 50% Deposit and 50% upon completion
12. SCHEDULE: To be determined. Anticipated timeline is 2-3 days.
1 a. Linear feet in specifications shown are approxiittates. Any actual footage above or
below said footage will be deemed to be included in estimate and not an 'add-on'.
2b. Any drywall, tile, concrete and black that may need to be removed will be patched
by others unless outlined in this estimate. We are not responsible for any landscaping
that may need to be removed for the pipe lining process.

professional manner to standard practices and building code in effect at the time of the
work. Any alternative or deviation from the above specifications involving extra
costs, will be executed only upon written orders, and will become an extra charge over
and above the estimate. All agreements contingent up any accidents or delays beyond
our control. Our workers are fully covered by Worker's Compensation insurance. We
may withdraw this proposal if not accepted within 30 days. This proposal does not
include any building permits or processing fees. Terms are due on receipt.

ACCEPT I ACCEPTANCE OF PROPOSAL:
By signing this proposal the undersigned hereby accepts this proposal and the same is
deemed a valid contract. The undersigned further agrees that the above prices,
specifications, and conditions are satisfactory and are hereby accepted. The
undersigned further authorizes KWIK PLUMBERS to do work as specified above.
Payment is to be made as outlined above. If payment is not made by the undersigned
as stated above, then the undersigned will be in default. The undersigned further
agrees that in the event that the undersigned is deemed in default and this matter is
placed with an attorney or collection agency for collections, the undersigned agrees to
pay all collection costs, including reasonable attorney and other agency fees. In
addition, the undersigned agrees to 1 112% service charge per month (or the maximum
allowed by law) on any unpaid balances to be in default andlor turned over for
collection.
