ORDER REVOKING BUSINESS TAX RECEIPTS

Receipt Nos.: 914 — Massage Establishment
915 — Individual Massage Therapist
This matter came before the Village Council for public hearing on September 12, 2013.
The Taxpayer neither appeared at the hearing nor contested the revocation of the Business Tax Receipts.
Upon consideration of the recommendation of Village Staff and based on the evidence
presented during the course of the hearing, the Village Council hereby finds that the revocation of
the Business Tax Receipts issued to the Taxpayer promotes the public peace, health, safety and
welfare of the neighborhood
Based on the foregoing, it is hereby ORDERED by the Village Council that:
1. Business Tax Receipt Nos. 914 and 915 issued to Hong Tang Life Therapy Massage
are hereby absolutely revoked pursuant to Section 17 -30 of the Village Code of Ordinances.
2. The Taxpayer shall not conduct any additional business activities at the Premises without
applying for a new Business Tax Receipt and complying with all state, county and Village requirements.
